NOTICE
In order to ensure the efficient and timely organization of medical examinations, PZU Zdrowie S.A. requests that employees strictly observe their scheduled appointment times and, in the event of a planned absence, to cancel appointments in advance so that the available time slot may be allocated to another individual.
The majority of initial and periodic medical examinations are carried out by an occupational medicine physician. Employees are referred for laboratory testing and specialist consultations in cases stipulated by applicable legal regulations:
Employee medical examinations are conducted on the basis of a referral containing a description of the work environment, based on occupational risk assessment documentation or the employee’s scope of duties.
Guidelines for supervisors to facilitate the completion of referrals:
Employees are required to undergo medical examinations before the expiry of the previous medical certificate (in the case of periodic examinations) or prior to commencing work (in the case of initial and follow-up examinations).
Initial and periodic medical examinations are conducted following telephone or personal registration by referred employee and scheduling the date and time for the commencement of the examination.
For follow-up examinations after sick leave lasting at least 30 days, employees are required to present a certificate confirming the completion of medical treatment.
